COVID-19 Policies

Our policies state that if you are displaying symptoms of an acute illness such as fever, cough, body aches, loss of taste or smell, please notify the office ahead of your visit.  A telehealth appointment may be arranged if deemed appropriate. If displaying any acute illness symptoms, please wear a mask when entering the office.

Confidentiality Statement

Your privacy is important to us. All medical records and interactions between providers and patients are completely confidential.  You will be provided and asked to sign a confidentiality statement when you establish care with our office.

 

Payments and Cancellation Policies

Payment is always due at the time of service including all co-pays.   We accept cash, checks, debit cards, visa and master card.

We accept most commercial insurance plans.  If you have a PPO style plan (these plans allow you to see doctors who are not part of your insurance company’s provider network), we can bill your insurance if your deductible has been met and/or prepare a health insurance claim form that you can submit to your insurance company to request reimbursement of your visit charges.  Any referrals for imaging or other testing will likely still be covered by your insurance even if made by an out-of-network provider.

We can never guarantee that your insurance company will reimburse you for your visits. You are ultimately responsible for the cost of your care at our office.  For patients with high deductible or Christian share plans or without insurance, we offer a 20% off same day cash price.   Please refer to the fee schedule.

We require a 48-hour cancellation notice.   If you cancel your appointment with less than 48 hours’ notice or fail to show for your appointment without notification you will be charged $25.
